		<description>Thanks Jacqueline. I like the idea to use index cards. It also reminded me to add a section for minors, as many of our first years are under 18. 

I was thinking overnight about whether we need all the person&#039;s  contact details or just a telephone number. It would take less time for them to sign.</description>
